Nature of Services

Well With Doc provides chiropractic and wellness services focused on the neuromusculoskeletal system and nervous system function. Services may include chiropractic adjustments addressing subluxation, movement and rehabilitation guidance, and wellness and lifestyle recommendations. These services are not a substitute for emergency medical care, primary care, or specialized medical treatment. If you are experiencing a medical emergency, you are instructed to call 911 or seek immediate medical attention immediately.


Direct Pay Model and Insurance

Well With Doc operates as a direct-pay practice and does not bill insurance for services rendered. Payment is due at the time of service unless otherwise agreed upon in writing. A superbill may be provided upon request; however, reimbursement from insurance providers is not guaranteed and remains the sole responsibility of the patient. Use of Health Savings Accounts or Flexible Spending Accounts is permitted when applicable, but eligibility and approval are determined by the patient’s plan.


Appointments, Cancellations, and Attendance

Appointments are reserved specifically for each patient and require mutual respect for scheduled time. A minimum of twenty-four hours notice is required for cancellations or rescheduling. Late cancellations and missed appointments may result in a fee up to the full cost of the scheduled service. Patients who arrive late may have their session shortened in order to maintain the schedule, and full session fees will still apply. Repeated missed appointments, late cancellations, or scheduling inconsistencies may result in restricted access to future appointments or discharge from care.


Communication and Access to Care

Well With Doc may provide patients with direct communication access for appropriate clinical support. This communication is not continuously monitored and should not be relied upon for urgent or emergency medical concerns. Messages are intended for non-emergent communication only. Patients are responsible for seeking immediate medical care when necessary, including contacting emergency services when appropriate.


Flare-Ups and Urgent Needs

Patients may experience changes in symptoms, including flare-ups or acute discomfort. While reasonable efforts may be made to accommodate urgent concerns, same-day or next-day appointments are not guaranteed and are dependent on availability. Patients are responsible for determining when emergency care is necessary and for seeking appropriate services outside of Well With Doc when required.


Scope and Limitations of Care

All care provided is based on clinical findings, professional judgment, and patient-reported information. Outcomes are not guaranteed, and individual responses to care may vary. Recommendations provided are intended to support optimal outcomes but are not mandatory. Patients have the right to accept or decline any aspect of care and are encouraged to seek additional medical opinions when appropriate.


Informed Consent

By receiving care at Well With Doc, patients acknowledge that they have been informed of the nature of chiropractic services and understand that there are inherent risks associated with care. These risks may include, but are not limited to, temporary soreness, discomfort, or aggravation of symptoms. Patients voluntarily consent to evaluation and treatment and understand that no specific results can be guaranteed.


Payment Terms and Financial Responsibility

Patients agree to be financially responsible for all services rendered. Payment is due at the time of service unless prior arrangements have been made. Outstanding balances may result in suspension or termination of care. All fees, pricing structures, and membership terms will be communicated clearly prior to initiation of services.


Membership Terms

Patients enrolled in membership plans agree to the terms outlined at the time of enrollment. Memberships may include recurring payments and defined visit structures. Monthly fees are due regardless of usage unless otherwise specified. Unused visits may expire based on the terms of the plan. Memberships are non-transferable unless explicitly stated. Well With Doc reserves the right to modify or discontinue membership offerings with appropriate notice.


Refund Policy

Services that have been rendered are non-refundable. Prepaid services or packages may be subject to specific terms disclosed at the time of purchase. Any exceptions to this policy are made at the sole discretion of Well With Doc.


Patient Responsibilities

Patients are responsible for providing accurate and complete health information, including updates to their condition as they occur. Patients agree to communicate openly regarding symptoms, changes, and concerns, and to participate in their care to the extent they are willing and able. Failure to provide accurate information or to communicate relevant changes may impact clinical outcomes and continuity of care.


Right to Refuse or Terminate Care

Well With Doc reserves the right to refuse or discontinue care at any time based on clinical judgment or non-clinical factors. These may include, but are not limited to, non-compliance with recommended care, inappropriate or disruptive behavior, repeated violations of office policies, or circumstances in which care is no longer deemed appropriate.


Use of Patient Information and Privacy

Well With Doc collects and maintains patient information for the purposes of providing clinical care, coordinating services, maintaining accurate records, and improving the quality of care delivered. This information may include personal identification details, health history, clinical findings, and communication records. Patient information is handled in accordance with applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA), where applicable. Information will not be shared with third parties without patient consent, except as required for treatment coordination, payment processing, healthcare operations, or as mandated by law. De-identified data may be used internally for quality improvement, education, or operational analysis. By receiving care, patients acknowledge and consent to the collection, storage, and appropriate use of their information as described.
